Reliance security staff failed to put a prisoner on suicide watch despite signs that he was at risk, an inquiry heard.
William Stewart died in hospital after trying to hang himself with his shoelaces in a cell at Glasgow Sheriff Court last August.
A post-mortem revealed old slash marks on his hand and forearms suggesting evidence of previous suicide attempts, but the Reliance guard watching him said he had not spotted the scars.
Mr Stewart's death is the first in Scotland of a prisoner while in the care of the controversial security firm.
